A password and a challenge question is just a password and a password.  This is one-factor verification.  Just like the extra number on the back of your credit card is still one-factor verification.  two-factor verification is not “two different pieces of knowledge” it’s “knowledge and a physical object.”  Swiping your debit card and then entering your PIN is two-factor verification.  entering the number on your card and then entering your PIN is one-factor verification.  Faux two factor verification is no more secure than one-factor, and provides a false sense of security which is more dangerous than knowing you have only one level of protection.
